Community health needs assessment data pack

Use when a nonprofit hospital's community benefit manager starts the triennial community health needs assessment. Pulls poverty, insurance, age and language from Census data, county unemployment from BLS, local hospital quality from CMS, and access to pharmacies and groceries from the map, then adds county open-data health datasets into a sourced secondary-data pack.

# Community health needs assessment data pack

For one hospital's service area (usually one to three counties). The CHNA needs a secondary-data section every reader can trace.

## 1. Demographics and coverage

1. Resolve counties and tracts with @[US Census Bureau · Geographies](alexandria:census-gov/census/geographies).
2. Pull poverty, uninsured share, age 65 and over, disability, language and broadband with @[US Census Bureau · Data](alexandria:census-gov/census/data) (acs/acs5; county and tract).
3. Add jobless rate with @[BLS · Bls unemployment rate](alexandria:bls-gov/economic-statistics/bls_unemployment_rate) (state_fips, county_fips).

## 2. Care and food access

4. List hospitals and their quality ratings with @[CMS Care Compare · Hospitals](alexandria:data-cms-gov/clinician-procedure-volume/hospitals) (state, city).
5. Map pharmacies and supermarkets with @[OpenStreetMap · Search elements](alexandria:openstreetmap-org/places/search_elements) (tags amenity=pharmacy, shop=supermarket; area_relation_id per county). Tracts with high poverty and none nearby are access gaps.

## 3. Local health data

6. Search the county or state open-data portal with @[Socrata · Search datasets](alexandria:socrata-com/open-data/search_datasets) (query overdose, births, chronic disease, behavioral risk) and pull rows with @[Socrata · Query dataset](alexandria:socrata-com/open-data/query_dataset) after reading columns with @[Socrata · Describe dataset](alexandria:socrata-com/open-data/describe_dataset).

## Write the data pack

- **Indicator table:** indicator, county value, state value, year, source.
- **Hot spots:** tracts combining high poverty, uninsured and poor access.
- **Draft priority list** for the community input phase, each tied to data.
- **Sources:** ACS tables, BLS series, CCNs, OSM ids, dataset ids. Say what could not be found.
